Early percutaneous dilatational tracheostomy leads to improved outcomes in critically ill medical patients as compared to delayed tracheostomy

چکیده
Subjects: All patients in the three medical ICUs who were intubated and mechanically ventilated for acute respiratory failure were screened and included if they were: >18 years old, projected to need mechanical ventilation >14 days, and had an initial APACHE II score >25. Specific exclusion criteria were established to ensure the safe performance of percutaneous tracheostomy (anatomical factors, evidence of potential prolonged bleeding, and PEEP >12 cm H2O).

OBJECTIVE The present prospective randomized trial compared surgical tracheostomy (ST) and percutaneous dilatational tracheostomy (PDT) in intensive care unit (ICU) patients in terms of outcomes and complications. METHODS Between January 2003 and December 2005 tracheostomies were performed on critically ill ICU patients in Medical Faculty Hospital in Prague, with a random allocation of 105 pa...

متن کاملTracheostomy: epidemiology, indications, timing, technique, and outcomes.
Tracheostomy is a common procedure performed in critically ill patients requiring prolonged mechanical ventilation for acute respiratory failure and for airway issues. The ideal timing (early vs late) and techniques (percutaneous dilatational, other new percutaneous techniques, open surgical) for tracheostomy have been topics of considerable debate. In this review, we address general issues reg...
متن کاملPercutaneous Dilatational Tracheostomy

However, during the past 20 years, the use of percutaneous dilatational tracheostomy has been increased and shown to be a feasible and safe procedure in critically ill patients. The purpose of this report is to review the percutaneous dilatational tracheostomy technique, describe the role of bronchoscopy as g...
متن کاملPercutaneous Dilatational Tracheostomy in Critically Ill Patients Taking Antiplatelet Agents
Background: Percutaneous dilatational tracheostomy (PDT) has been considered as an alternative to surgical tracheostomy in intensive care units (ICU), and is widely used for critically ill patients who need prolonged mechanical ventilation. Few studies have reported on PDT performed in critically ill patients taking antiplatelet agents. Our goals are to assess not only the feasibility and safet...
